Orthognathic Breakout Session: BSSO Technique

The mandibular ramus sagittal split osteotomy is one of the most versatile procedures in orthognathic surgery. A modified technique will be presented that allows predictable sagittal splitting of the ramus/body for mandibular advancement, set-back, or asymmetry correction while minimizing complications. The modified surgical design allows application of rigid fixation with a small bone plate, no matter how far the mandibular advancement or set-back. The proximal segment and positioning of the condyle within the fossa can be controlled to prevent condylar sag as well as elimination of the esthetic defect at the gonial notch area commonly seen with the traditional BSSO technique. The use of the inferior border saw improves the ease of splitting, significantly decreases unfavorable splits and potential injury to the inferior alveolar nerve, as well as allows removal of the 3rd molars at the same time. Reasons for relapse and prevention will be discussed as well as outcome data.
