Dental Implant Breakout Session: Immediate Function Dental Implant treatment for the Edentulous Maxilla

The edentulous maxilla is a challenge to treat, particularly in late stage atrophy. There are two approaches for treating the maxilla, one of bone graft reconstruction with simultaneous or delayed placement of axial implants or the use of tilted implants with bone removal and leveling for immediate loading. Both approaches can be used for immediate function provided there is adequate primary stability of implants. In cases of extreme atrophy extramaxillary implant locations can be considered such at the pterygoid plate or the zygoma to anchor implants. Zygomatic implants have been used delayed or immediate depending on the setting. Occasionally four zygomatic implants are used, two on each side, to gain support for a restorative appliance in settings of absence of bone. The use of the various implant schemes advocated in the current literature are trending away from extensive bone graft reconstruction such as sinus grafts and onlay bone grafts. Also, the use of delayed placement protocols is now commonly supplanted because of the desire for immediate function treatment.
- Explain surgical prosthetic objectives for the implant restorative effort chosen;
- Discuss prosthetic demands for bone removal and how to obtain adequate insertion torque for immediate function for the all on four treatment approach;
- Utilize prosthetic templates and gingivoalveolar esthetic procedures for and emergence profile complete arch restoration; and
- Collaborate with the prosthodontist/restorative dentist for alternative treatments using zygomatic implants.
