Posterior Immediate Implant Placement with Simultaneous Crestal Approach Maxillary Sinus Bone Graft and Flapless Extraction

There are several clinical studies that have shown successful clinical outcomes in immediate posterior implant placements and posterior implant placement with simultaneous maxillary sinus bone graft procedure. Fugazzotto & Hains (Compendium 2012 Jul-Aug; 34(7):518-28) has shown success in immediate maxillary posterior implants placed with simultaneous maxillary sinus augmentation procedure at time of extraction. In addition, there are several clinical studies that have shown successful outcomes in immediate implant placement in a tooth with periapical pathology/infection. I will show clinical cases of immediate posterior implant placement with simultaneous maxillary sinus bone graft via crestal approach at the time of molar extraction. Flapless extractions were performed and immediate implants were placed in a single stage (healing abutments placed) without additional soft tissue grafting. These cases had successful outcomes even with periapical pathology and/or inflammation/thickening of the maxillary sinus membrane.
