Enucleation of the Extensive Cyst on Mandible via Sagittal Split Ramus Osteotomy : Case Report

Odontogenic keratocyst(OKC) frequently occurs on mandible, it can grow as occupying most of mandible body and ramus without any symptom. Marsupilalization procedure has a limit for reducing them because of their large size. Moreover, traditional cyst enucleation results in so much bone loss that it increase the risk of mandible fracture and lead to facial deformity. As the another method, we can use sagittal split ramus osteotomy for enucleation of the extensive cyst. Recently, sagittal split ramus osteotomy is frequently used for maxillofacial plastic surgery. It gets rid of the cyst via mandible osteotomy sagittaly without extensive bone loss and reunions proximal and distal osteocommas with screw and plate. This technic minimizes the loss of cortical bone, so it prevents facial deformity and reduces the risk of fracture. On this, we report 4 patients whose extensive cyst on mandible were enucleated via sagittal split ramus osteotomy in Dept. of Oral and Maxillofacial Surgery, Pusan National University Dental Hospital in 2012.
