Synopsis
The soft tissue paradigm shift was a change in orthognathic planning from one based on traditional cephalometrics to one analyzing a facial profile in repose. An exciting newer method is presented that focuses on patients’ smiling profiles to discern dentofacial imbalance and establish clear goals for predictably harmonious surgical outcomes.

- Explain why a diagnosis based on traditional cephalometrics and the soft tissue repose profile is inadequate;
- Explain how the analysis of a smiling profile permits effective treatment planning for facial harmony; and
- Discuss how the facial planes defined in a new way can be used as landmarks to quantify planned changes to the jaws.
