Synopsis
Two diverging philosophies relative to management of orthognathic surgery patients presenting with pre-existing TMJ internal derangement will be presented. One will recommend surgical correction of the TMJ concomitantly with orthognathic surgery, while the other will recommend a more conservative and/or staged approach. Evidence-based literature and long-term personal experience will be discussed.
Learning Objectives
- Distinguish those patients who do not require both types of surgery.
- Explain the debatable relationship between TMJ and orthognathic surgery.
- Discuss whether both types of surgery should be done concomitantly or separately. If concomitantly, how to sequence surgery? If separately, which type of surgery to do first?
