Synopsis
Hospital-based orthognathic surgery is conventionally used for the predictable management of skeletal malocclusions. Recently, selective alveolar decortication (SAD) and accelerated orthodontics have been proposed as an office-based alternative. This presentation will demonstrate success and failure with the goal of defining specific indications for predictable use of this technique.

- Describe indications, contraindications, and limitations of selective alveolar decortication.
- Explain the armamentarium required and surgical technique.
- Discuss the regional acceleratory phenomenon as related to surgery in the maxilla and mandible, and pearls for communication with orthodontists.
