    Synopsis

    Hospital-based orthognathic surgery is conventionally used for the predictable management of skeletal malocclusions. Recently, selective alveolar decortication (SAD) and accelerated orthodontics have been proposed as an office-based alternative. This presentation will demonstrate success and failure with the goal of defining specific indications for predictable use of this technique.

    Learning Objectives
    1. Describe indications, contraindications, and limitations of selective alveolar decortication.
    2. Explain the armamentarium required and surgical technique.
    3. Discuss the regional acceleratory phenomenon as related to surgery in the maxilla and mandible, and pearls for communication with orthodontists.
